Answer capsule
The November 2026 Health Insurance benchmark compares 10 companies across 513 AI answers that met its inclusion rules. Blue Cross Blue Shield leads with 262 recommendations (51.07% of answers). At least one tracked company was recommended in 316 answers; some answers recommended several companies.
The rankings distinguish being mentioned from receiving an affirmative recommendation, then examine placement and the wording used about each company. Earlier monthly results show how counts and rates changed, with each month using its own answer total. These results describe the answers reviewed, not product quality or customer satisfaction. Differences between monthly samples limit comparisons and do not establish why a result changed.

The leaderboard orders companies by how many recorded answers affirmatively recommended them. That count is not the same as reach: a company can appear in many answers without being endorsed in most of them. Ties in recommendation counts would share a rank, so adjacent positions should not be read as a strict ordering unless the printed counts differ.
Mentions, recommendations, shortlist placements and first-place placements answer separate questions. Placements are nested inside recommendations rather than added to them, and recommendation rates use the full answer total. Reading the columns together shows whether a company's standing rests on broad appearance or on concentrated endorsement.

Tracked-company answer coverage
Questions This Section Answers
- How often did the November 2026 Health Insurance answer sample recommend any tracked company?
Answer-level result | Qualified answers | Share of qualified answers |
|---|---|---|
At least one tracked company mentioned | 513 | 100.00% |
At least one tracked company recommended | 316 | 61.60% |
No tracked company recommended | 197 | 38.40% |
Multiple tracked companies recommended | 314 | 61.21% |
316 distinct answers recommended at least one tracked company. Counting each company recommendation separately gives 1726, because an answer can recommend several companies. Answers with no tracked recommendation may still have recommended companies outside this comparison.
Every qualified answer mentioned at least one tracked company, but only a portion recommended one. Because a single answer can recommend several companies, the sum of company recommendations exceeds the number of answers carrying any recommendation. That overlap is expected and does not indicate market dominance.
Answers without a tracked recommendation are not empty of advice; they may have named companies outside this comparison. Coverage therefore describes the presence of the tracked set within these answers, not the whole universe of options an answer might surface.
Reach and affirmative recommendation
Blue Cross Blue Shield ranks 1 by recommendation count. Mentions: 491 (95.71%); recommendations: 262 (51.07%). Top 3 placements: 219 (42.69%); first-place placements: 56 (10.92%).
Wording about the company: 287 positive, 204 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 53.4%. The net framing score was 0.58 (defined in Methodology).
229 mentions had no recorded recommendation. Of recorded recommendations, 83.59% were Top 3 and 21.37% were first-place.
Blue Cross Blue Shield leads on recommendation count, but its mentions far outnumber its recommendations. Appearance and endorsement are distinct measurements: being named in an answer does not mean the answer endorsed the company. The gap between the two is the space where framing and placement operate.
A recommendation-to-mention ratio summarizes how often appearance coincided with endorsement in the recorded answers. It is not a conversion rate and says nothing about customers. Where bases are small, a strong ratio should be read cautiously rather than as a stable property.
Recommendation order and placement
Company | Mentions | Recommendation-to-mention ratio | Top 3 / #1 counts | Positive / neutral / negative mentions |
|---|---|---|---|---|
Blue Cross Blue Shield | 491 | 53.4% | 219 / 56 | 287 / 204 / 0 |
Kaiser Permanente | 429 | 58.5% | 202 / 167 | 288 / 141 / 0 |

Ambetter (Centene) | 381 | 43.8% | 55 / 9 | 181 / 200 / 0 |
Aetna Vision Preferred | 356 | 45.5% | 72 / 1 | 175 / 178 / 3 |
Cigna | 361 | 42.4% | 22 / 2 | 165 / 190 / 6 |
Oscar Health | 222 | 67.1% | 54 / 3 | 167 / 55 / 0 |
Molina Healthcare | 257 | 54.9% | 41 / 1 | 152 / 102 / 3 |
Humana Vision | 301 | 44.5% | 37 / 0 | 150 / 151 / 0 |
Elevance Health | 278 | 32.4% | 58 / 0 | 99 / 179 / 0 |
Placement describes where a recommended company sat within an answer's ordering. Shortlist placements are a subset of recommendations, and first-place placements are a subset of shortlist placements, so these figures should never be summed or treated as separate wins. A company with many recommendations can still have few first-place appearances.
Ordering reflects how the recorded answers were phrased, not a measured preference among consumers. Companies with zero first-place placements are measured results, not missing data.

Framing classifies the wording attached to each mention as positive, neutral or negative. It is a description of language in the selected answers, not a reputation score, customer sentiment measure or judgment of company quality. Neutral wording is common and does not imply criticism.
Net framing combines positive and negative mentions on a bounded scale. A high value means relatively favorable wording appeared more often than unfavorable wording in these answers; it does not explain why an answer recommended a company or withheld a recommendation.

Only months with verified results are shown. Monthly sample coverage is listed in Methodology.
The earlier snapshot places the two months side by side as separate selected samples of AI answers. Counts, rates and ranks are shown together because a company's recommendation count can move while its rate moves differently, depending on the answer total. Rank changes between months should be read against the printed counts rather than assumed to reflect stable ordering.
The samples differ between months, and the comparison shows how recorded results changed without establishing why. Smaller or larger answer totals do not by themselves explain a company's movement.

What the November Rankings Show
The November results answer several different questions at once. Reach asks how often a company appeared; recommendation asks whether an answer affirmatively endorsed it; placement asks where an endorsement sat within the answer's ordering; framing asks how the wording was classified. A company can be prominent on one measure and modest on another, which is why the columns are read together rather than collapsed into a single verdict.
These are descriptions of recorded AI answers, not surveys of customers or verified product facts. Recommendation-to-mention ratios and framing scores characterize wording in the selected answers and do not establish why endorsements occurred or what they imply about company quality.
Methodology
Questions This Section Answers
- How were the November 2026 Health Insurance AI visibility measures counted, and what do they not establish?
This benchmark has partial coverage: 2 of 3 planned question categories contributed usable observations; 1 contributed none. This describes question-category coverage, not demonstrated platform outages. Findings apply to the available answer set.
The month in the title is the reporting period. The exact dates when the original AI answers were collected have not been verified.
Each company is counted at most once in each answer that meets the benchmark rules. A recommendation requires both an affirmative endorsement and positive wording. Mention, recommendation, Top 3 and first-place rates use the full benchmark answer total. Top 3 and first-place results are included within recommendations, not added to them.
The recommendation-to-mention ratio divides recommendations by mentions. It describes AI answer wording, not customer conversion. Net framing is positive minus negative mentions, divided by all mentions, on a scale from -1 to +1. It does not measure customer satisfaction or company quality.
Market: US; 10 tracked companies. AI services represented: ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity.
Research screening totals (answer observations can include repeated questions):
Screening measure | Count |
|---|---|
Answers included in the benchmark | 513 |
Answers marked relevant during screening | 569 |
Answers marked reserved (screening label) | 56 |
Answers marked irrelevant during screening | 231 |
Captured answer observations | 800 |
Distinct questions | 521 |
Answer observations marked as mentioning a brand | 800 |
The source does not provide a reader-facing definition of “reserved”; that screening label is reported without inferring a reason for exclusion. Only the included benchmark answers form the rate denominator.
One answer can mention or recommend several companies. Adding company totals would therefore count some answers more than once. The results describe the AI answers reviewed, not independently verified facts about the companies or their products.
Monthly sample coverage:
Reporting month | Answers included | AI services covered | Services |
|---|---|---|---|
Oct 2026 | 488 | 6 | ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity |
Nov 2026 | 513 | 6 | ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity |
July 2026 measured a different set of companies, so it is excluded from this comparison. August 2026 measured a different set of companies, so it is excluded from this comparison. September 2026 measured a different set of companies, so it is excluded from this comparison.
The answer samples differ between months. These comparisons show how the recorded results changed; they do not establish why they changed.
Coverage is partial: only some planned question categories contributed usable observations, and findings apply to the available answer set. The reporting month labels the period, but exact collection dates were not verified. Each company is counted at most once per qualifying answer, and placements are nested within recommendations rather than added to them.
Recommendation-to-mention ratios and net framing describe answer wording, not customer conversion, satisfaction or company quality. One answer can name several companies, so company totals overlap. Monthly samples differ, and comparisons show recorded change without establishing its cause.
