Answer capsule
The November 2026 Medicare Supplement Insurance benchmark compares 10 companies across 95 AI answers that met its inclusion rules. UnitedHealthcare Vision leads with 73 recommendations (76.84% of answers). At least one tracked company was recommended in 76 answers; some answers recommended several companies.
The rankings distinguish being mentioned from receiving an affirmative recommendation, then examine placement and the wording used about each company. Earlier monthly results show how counts and rates changed, with each month using its own answer total. These results describe the answers reviewed, not product quality or customer satisfaction. Differences between monthly samples limit comparisons and do not establish why a result changed.

The leaderboard orders companies by how many reviewed AI answers affirmatively recommended them. That count is not the same as appearing in an answer, and it is not the same as being placed near the top of a list. A company can be mentioned often yet recommended less often, so the columns answer different questions.
Ties are decided by equal recommendation counts, not by framing or placement. Top-three and first-place results sit inside the recommendation set rather than beside it, so they should be read as position within endorsements, not as extra wins.

Tracked-company answer coverage
Questions This Section Answers
- How often did the November 2026 Medicare Supplement Insurance answer sample recommend any tracked company?
Answer-level result | Qualified answers | Share of qualified answers |
|---|---|---|
At least one tracked company mentioned | 95 | 100.00% |
At least one tracked company recommended | 76 | 80.00% |
No tracked company recommended | 19 | 20.00% |
Multiple tracked companies recommended | 75 | 78.95% |
76 distinct answers recommended at least one tracked company. Counting each company recommendation separately gives 359, because an answer can recommend several companies. Answers with no tracked recommendation may still have recommended companies outside this comparison.
Almost every reviewed answer named at least one tracked company, and most recommended one. Many answers recommended several companies at once, which is why company-level recommendation totals exceed the number of answers carrying any recommendation.
This overlap means coverage describes how often tracked names appear across answers, not market share or dominance. An answer with no tracked recommendation may still have named companies outside this comparison, so absence here is not evidence that no recommendation occurred.
Reach and affirmative recommendation
UnitedHealthcare Vision ranks 1 by recommendation count. Mentions: 90 (94.74%); recommendations: 73 (76.84%). Top 3 placements: 63 (66.32%); first-place placements: 38 (40.00%).
Wording about the company: 73 positive, 17 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 81.1%. The net framing score was 0.81 (defined in Methodology).
17 mentions had no recorded recommendation. Of recorded recommendations, 86.30% were Top 3 and 52.05% were first-place.
Mention measures whether a company appeared in an answer; recommendation requires an affirmative endorsement with positive wording. UnitedHealthcare Vision leads on recommendation count, but its mentions outnumber its recommendations, so reach and endorsement diverge even for the top name.
A tie in recommendation counts is a tie, not a hidden winner. The gap between mentions and recommendations shows that being named is a weaker signal than being endorsed, and neither establishes why an answer took that form.
Recommendation order and placement
Company | Mentions | Recommendation-to-mention ratio | Top 3 / #1 counts | Positive / neutral / negative mentions |
|---|---|---|---|---|
UnitedHealthcare Vision | 90 | 81.1% | 63 / 38 | 73 / 17 / 0 |
Cigna | 77 | 77.9% | 19 / 0 | 60 / 16 / 1 |
Mutual of Omaha | 64 | 92.2% | 40 / 5 | 59 / 5 / 0 |
State Farm | 64 | 84.4% | 40 / 12 | 54 / 10 / 0 |
Humana Vision | 49 | 67.3% | 12 / 2 | 33 / 16 / 0 |
Aetna Vision Preferred | 47 | 68.1% | 9 / 0 | 32 / 15 / 0 |
Blue Cross Blue Shield | 46 | 60.9% | 6 / 2 | 28 / 18 / 0 |
Anthem Blue View Vision | 31 | 61.3% | 8 / 4 | 19 / 12 / 0 |
Bankers Life | 1 | 100.0% | 0 / 0 | 1 / 0 / 0 |
Colonial Penn | 0 | Unavailable | 0 / 0 | 0 / 0 / 0 |
Placement describes where a recommended company sat within an answer's ordering. Top-three and first-place counts are subsets of recommendations, not separate achievements, and they share the same answer denominator as the other rates.
A high recommendation-to-mention ratio means most mentions of that company also carried an endorsement in the recorded answers. It does not describe customers, sales or conversion, and framing wording is not evidence of company quality or of why placement occurred.
Classified framing in context
Company | Positive | Neutral | Negative |
|---|---|---|---|
UnitedHealthcare Vision | 73 | 17 | 0 |
Cigna | 60 | 16 | 1 |
Mutual of Omaha | 59 | 5 | 0 |
State Farm | 54 | 10 | 0 |
Humana Vision | 33 | 16 | 0 |
Aetna Vision Preferred | 32 | 15 | 0 |
Blue Cross Blue Shield | 28 | 18 | 0 |
Anthem Blue View Vision | 19 | 12 | 0 |
Bankers Life | 1 | 0 | 0 |
Colonial Penn | 0 | 0 | 0 |
Framing classifies the wording used about a company in the reviewed answers as positive, neutral or negative. Most tracked companies drew largely positive or neutral wording, with negative wording rare in the recorded set.
Neutral wording is not a negative signal, and positive wording does not establish that a recommendation was earned or deserved. Framing reflects how answers phrased things, not customer sentiment, reputation or the reasons behind recommendation differences.

Questions This Section Answers
- What do the November 2026 AI answer results show about UnitedHealthcare Vision mentions, recommendations and placement?
UnitedHealthcare Vision ranks 1 by recommendation count. Mentions: 90 (94.74%); recommendations: 73 (76.84%). Top 3 placements: 63 (66.32%); first-place placements: 38 (40.00%).
Wording about the company: 73 positive, 17 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 81.1%. The net framing score was 0.81 (defined in Methodology).

Wording about it was positive or neutral, with no negative mentions recorded. The prior-month comparison shows lower counts and a lower recommendation rate, but the monthly samples differ, so the change is reported rather than explained.
Cigna
Questions This Section Answers
- What do the November 2026 AI answer results show about Cigna mentions, recommendations and placement?
Cigna ranks 2 by recommendation count. Mentions: 77 (81.05%); recommendations: 60 (63.16%). Top 3 placements: 19 (20.00%); first-place placements: 0 (0.00%).

Cigna ranks second on recommendation count with wide mention reach, but its endorsements rarely reach the top of an answer's ordering and none were recorded in first place. That combination separates being recommended from being placed prominently.
Wording was mostly positive with a single negative mention. Counts and the recommendation rate were lower than the previous month, a recorded comparison across different samples that does not establish why the shift occurred.
Mutual of Omaha
Questions This Section Answers
- What do the November 2026 AI answer results show about Mutual of Omaha mentions, recommendations and placement?

Mutual of Omaha appears in a large share of the recorded answers and receives affirmative endorsement in most of those appearances. Its wording is overwhelmingly positive, with neutral phrasing rare and no negative framing recorded. Endorsements frequently sit within the top three, while first-place positions form a smaller subset.
The recommendation-to-mention ratio describes the recorded answers, not customers or sales. Both counts and the recommendation rate were lower than the prior month; because the monthly samples are separate, that comparison records a change without identifying its cause.

Wording was positive or neutral with no negative mentions. Its recommendation rate was close to the previous month even as raw counts fell, and the differing samples mean the movement is reported, not explained.

Wording was positive or neutral, with a notable share of neutral mentions. Its recommendation rate was nearly steady against the prior month while counts fell, a comparison across different samples that does not establish cause.

Aetna Vision Preferred appears in roughly half the recorded answers and receives endorsement in a smaller portion of them. Its endorsements are seldom placed in the top three, and no first-place positions were recorded. Wording was positive or neutral throughout, with no negative framing.
The recommendation rate was close to the prior month while counts declined. These are separate monthly samples, so the comparison records the two snapshots and its limits rather than explaining the movement.

Wording was positive or neutral, with neutral mentions forming a large share. Its recommendation rate was markedly lower than the prior month, a change recorded across different samples that does not establish why it occurred.

Wording was positive or neutral. Its recommendation rate was higher than the prior month while counts were similar, and the differing samples mean the shift is reported without a causal reading.

Only months with verified results are shown. Monthly sample coverage is listed in Methodology.
The monthly tables show separate samples of AI answers, each with its own answer total, so counts and rates should be read together rather than compared as if sampling were identical. A lower count can accompany a steady rate, and a higher rate can appear alongside fewer answers.
Rank positions are shown for context only. The comparisons describe how recorded results differed across months; they do not establish rank stability, equivalent sampling or the reasons behind any movement.

What the November Rankings Show
The November results answer several distinct questions. Reach asks how often a company appeared; recommendation asks whether an answer affirmatively endorsed it; placement asks where an endorsement sat within an answer's ordering; framing asks how the wording was classified. A company can be strong on one and weaker on another.
Reading them together shows that visibility and endorsement are not interchangeable, and that prominent placement is a subset of recommendation rather than a separate win. None of these measures describes customers, sales or company quality, and the recorded patterns do not explain why answers took the form they did.
Methodology
Questions This Section Answers
- How were the November 2026 Medicare Supplement Insurance AI visibility measures counted, and what do they not establish?
This benchmark has partial coverage: 2 of 3 planned question categories contributed usable observations; 1 contributed none. This describes question-category coverage, not demonstrated platform outages. Findings apply to the available answer set.
The month in the title is the reporting period. The exact dates when the original AI answers were collected have not been verified.
Each company is counted at most once in each answer that meets the benchmark rules. A recommendation requires both an affirmative endorsement and positive wording. Mention, recommendation, Top 3 and first-place rates use the full benchmark answer total. Top 3 and first-place results are included within recommendations, not added to them.
The recommendation-to-mention ratio divides recommendations by mentions. It describes AI answer wording, not customer conversion. Net framing is positive minus negative mentions, divided by all mentions, on a scale from -1 to +1. It does not measure customer satisfaction or company quality.
Market: US; 10 tracked companies. AI services represented: ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity.
Research screening totals (answer observations can include repeated questions):
Screening measure | Count |
|---|---|
Answers included in the benchmark | 95 |
Answers marked relevant during screening | 182 |
Answers marked reserved (screening label) | 87 |
Answers marked irrelevant during screening | 618 |
Captured answer observations | 800 |
Distinct questions | 575 |
Answer observations marked as mentioning a brand | 800 |
The source does not provide a reader-facing definition of “reserved”; that screening label is reported without inferring a reason for exclusion. Only the included benchmark answers form the rate denominator.
One answer can mention or recommend several companies. Adding company totals would therefore count some answers more than once. The results describe the AI answers reviewed, not independently verified facts about the companies or their products.
Monthly sample coverage:
Reporting month | Answers included | AI services covered | Services |
|---|---|---|---|
Sep 2026 | 299 | 6 | ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity |
Oct 2026 | 255 | 6 | ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity |
Nov 2026 | 95 | 6 | ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity |
July 2026 measured a different set of companies, so it is excluded from this comparison. August 2026 measured a different set of companies, so it is excluded from this comparison.
The answer samples differ between months. These comparisons show how the recorded results changed; they do not establish why they changed.
Coverage is partial: only some planned question categories contributed usable answers, so findings apply to the available answer set, and the exact collection dates were not verified. Each company is counted at most once per answer, and one answer can name several companies, so company totals overlap.
Recommendation requires affirmative endorsement with positive wording. Top-three and first-place results sit inside recommendations, not alongside them. Ratios and framing scores describe answer wording, not customer conversion, satisfaction or company quality. Monthly samples differ, so comparisons show recorded change without establishing cause.
