Answer capsule

The November 2026 Long-Term Care Insurance benchmark compares 10 companies across 235 AI answers that met its inclusion rules. New York Life leads with 176 recommendations (74.89% of answers). At least one tracked company was recommended in 213 answers; some answers recommended several companies.

The rankings distinguish being mentioned from receiving an affirmative recommendation, then examine placement and the wording used about each company. Earlier monthly results show how counts and rates changed, with each month using its own answer total. These results describe the answers reviewed, not product quality or customer satisfaction. Differences between monthly samples limit comparisons and do not establish why a result changed.

The leaderboard orders companies by how many answers affirmatively recommended them, so it measures recorded endorsement rather than overall presence. A company can appear often yet be recommended less often, and the gap between mention and recommendation is itself informative. Ties in recommendation counts would share a rank; none are reported here. Placement and framing are separate readings, not extra points added to the recommendation total.

213 distinct answers recommended at least one tracked company. Counting each company recommendation separately gives 737, because an answer can recommend several companies. Answers with no tracked recommendation may still have recommended companies outside this comparison.

Nearly every included answer named at least one tracked company, and most recommended at least one. Because a single answer can endorse several companies, company-level totals exceed the number of answers, so they should not be summed as if each answer counted once. Answers without a tracked recommendation may still have named companies outside this comparison, which limits any claim of full market coverage.

Reach and affirmative recommendation

New York Life ranks 1 by recommendation count. Mentions: 199 (84.68%); recommendations: 176 (74.89%). Top 3 placements: 134 (57.02%); first-place placements: 20 (8.51%).

Wording about the company: 182 positive, 17 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 88.4%. The net framing score was 0.91 (defined in Methodology).

23 mentions had no recorded recommendation. Of recorded recommendations, 76.14% were Top 3 and 11.36% were first-place.

Mention means the company appeared in an answer; recommendation means the answer affirmatively endorsed it. New York Life leads on recommendation count, but its mentions outnumber its recommendations, so presence and endorsement are not interchangeable. The share of its mentions that carried a recommendation is high, yet a substantial minority of appearances stopped short of endorsement.

Top-three and first-place results are nested inside recommendations, not separate wins. A company recommended in many answers may still rarely appear near the top of a list, and the reverse can hold. These placements describe ordering within recorded answers; they do not measure customer preference, sales, or conversion of any kind.

Framing classifies the wording used about a company in the selected answers. Most mentions here are positive, with neutral wording common and negative wording rare. Positive or neutral phrasing does not explain why a recommendation was or was not recorded, and it is not a measure of customer sentiment or company quality.

Company results in detail

New York Life

Questions This Section Answers

  • What do the November 2026 AI answer results show about New York Life mentions, recommendations and placement?

New York Life ranks 1 by recommendation count. Mentions: 199 (84.68%); recommendations: 176 (74.89%). Top 3 placements: 134 (57.02%); first-place placements: 20 (8.51%).

Wording about the company: 182 positive, 17 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 88.4%. The net framing score was 0.91 (defined in Methodology).

23 mentions had no recorded recommendation. Of recorded recommendations, 76.14% were Top 3 and 11.36% were first-place.

From October 2026 to November 2026, New York Life's mention count fell from 209 to 199, while its recommendation count stayed at 176. The recommendation rate was 67.69% and 74.89% respectively. The count was unchanged: the rate difference comes from dividing it by 260 answers in October 2026 and 235 in November.

New York Life leads on recommendation count and appears in most answers, with a high share of its mentions carrying an endorsement. Its placements skew toward the top three rather than first place, and its wording is almost entirely positive. The prior month shows mentions easing while recommendations held steady; the rate moved because the answer base changed, not because endorsement behavior did.

Mutual of Omaha

Questions This Section Answers

  • What do the November 2026 AI answer results show about Mutual of Omaha mentions, recommendations and placement?

Mutual of Omaha ranks 2 by recommendation count. Mentions: 171 (72.77%); recommendations: 156 (66.38%). Top 3 placements: 99 (42.13%); first-place placements: 61 (25.96%).

Wording about the company: 160 positive, 11 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 91.2%. The net framing score was 0.94 (defined in Methodology).

15 mentions had no recorded recommendation. Of recorded recommendations, 63.46% were Top 3 and 39.10% were first-place.

From October 2026 to November 2026, Mutual of Omaha's mention count fell from 198 to 171, while its recommendation count fell from 173 to 156. The recommendation rate was 66.54% and 66.38% respectively.

Mutual of Omaha combines broad reach with a high recommendation-to-mention ratio, and an unusually large share of its endorsements land in first place. Wording is overwhelmingly positive with no negative mentions recorded. Both mentions and recommendations eased from the prior month while the recommendation rate held roughly level, a comparison that does not establish why the counts moved.

Northwestern Mutual

Questions This Section Answers

  • What do the November 2026 AI answer results show about Northwestern Mutual mentions, recommendations and placement?

Northwestern Mutual ranks 3 by recommendation count. Mentions: 160 (68.09%); recommendations: 140 (59.57%). Top 3 placements: 80 (34.04%); first-place placements: 38 (16.17%).

Wording about the company: 145 positive, 15 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 87.5%. The net framing score was 0.91 (defined in Methodology).

20 mentions had no recorded recommendation. Of recorded recommendations, 57.14% were Top 3 and 27.14% were first-place.

From October 2026 to November 2026, Northwestern Mutual's mention count rose from 151 to 160, while its recommendation count rose from 132 to 140. The recommendation rate was 50.77% and 59.57% respectively.

Northwestern Mutual sits in the upper group on recommendation count, with most of its mentions endorsed and a meaningful first-place presence. Positive wording dominates and no negative mentions appear. Both mentions and recommendations rose from the prior month, and the recommendation rate also increased; the changed sample does not explain that movement.

Nationwide

Questions This Section Answers

  • What do the November 2026 AI answer results show about Nationwide mentions, recommendations and placement?

Nationwide ranks 4 by recommendation count. Mentions: 168 (71.49%); recommendations: 135 (57.45%). Top 3 placements: 87 (37.02%); first-place placements: 23 (9.79%).

Wording about the company: 138 positive, 30 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 80.4%. The net framing score was 0.82 (defined in Methodology).

33 mentions had no recorded recommendation. Of recorded recommendations, 64.44% were Top 3 and 17.04% were first-place.

From October 2026 to November 2026, Nationwide's mention count rose from 160 to 168, while its recommendation count fell from 138 to 135. The recommendation rate was 53.08% and 57.45% respectively.

Nationwide's recorded answers place it fourth by recommendation count, with mentions appearing in a large majority of the answers and recommendations in a somewhat smaller share. Wording around the company was overwhelmingly positive, with the remainder neutral and none negative, so framing in these answers was favorable without exception.

Recommendations were frequently accompanied by shortlist placement, and first-place placements formed a smaller subset of those. Mentions rose slightly between the two selected months while recommendations fell, and the recommendation rate moved in the opposite direction; these are separate recorded comparisons, and the change does not establish why either count moved.

Transamerica

Questions This Section Answers

  • What do the November 2026 AI answer results show about Transamerica mentions, recommendations and placement?

Transamerica ranks 5 by recommendation count. Mentions: 64 (27.23%); recommendations: 56 (23.83%). Top 3 placements: 22 (9.36%); first-place placements: 1 (0.43%).

Wording about the company: 59 positive, 4 neutral, 1 negative mentions. The proportion of mentions that also included a recommendation was 87.5%. The net framing score was 0.91 (defined in Methodology).

8 mentions had no recorded recommendation. Of recorded recommendations, 39.29% were Top 3 and 1.79% were first-place.

From October 2026 to November 2026, Transamerica's mention count rose from 62 to 64, while its recommendation count rose from 51 to 56. The recommendation rate was 19.62% and 23.83% respectively.

Transamerica occupies a middle position, with a recommendation-to-mention ratio close to the leaders but far fewer total appearances. Its endorsements rarely reach first place, and one negative mention is recorded alongside mostly positive wording. Both mentions and recommendations rose from the prior month; the small base means the ratio should be read with that scale in mind.

Lincoln Financial

Questions This Section Answers

  • What do the November 2026 AI answer results show about Lincoln Financial mentions, recommendations and placement?

Lincoln Financial ranks 6 by recommendation count. Mentions: 47 (20.00%); recommendations: 40 (17.02%). Top 3 placements: 4 (1.70%); first-place placements: 0 (0.00%).

Wording about the company: 40 positive, 7 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 85.1%. The net framing score was 0.85 (defined in Methodology).

7 mentions had no recorded recommendation. Of recorded recommendations, 10.00% were Top 3 and 0.00% were first-place.

From October 2026 to November 2026, Lincoln Financial's mention count rose from 41 to 47, while its recommendation count rose from 33 to 40. The recommendation rate was 12.69% and 17.02% respectively.

Lincoln Financial is mentioned in a minority of answers and endorsed in most of those mentions, but its placements are almost never in the top three and never first. Wording is largely positive with some neutral mentions. Both mentions and recommendations rose from the prior month, though the counts remain modest and the comparison does not establish a trend.

Thrivent

Questions This Section Answers

  • What do the November 2026 AI answer results show about Thrivent mentions, recommendations and placement?

Thrivent ranks 7 by recommendation count. Mentions: 22 (9.36%); recommendations: 18 (7.66%). Top 3 placements: 1 (0.43%); first-place placements: 0 (0.00%).

Wording about the company: 18 positive, 4 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 81.8%. The net framing score was 0.82 (defined in Methodology).

4 mentions had no recorded recommendation. Of recorded recommendations, 5.56% were Top 3 and 0.00% were first-place.

From October 2026 to November 2026, Thrivent's mention count stayed at 22, while its recommendation count stayed at 18. The recommendation rate was 6.92% and 7.66% respectively. The count was unchanged: the rate difference comes from dividing it by 260 answers in October 2026 and 235 in November.

Thrivent appears in a small slice of answers, and most of those appearances carry an endorsement, though top-three placements are rare and first place is absent. Wording is mostly positive. Mentions and recommendations were unchanged from the prior month, while the recommendation rate shifted only because the answer base differed.

National Guardian Life

Questions This Section Answers

  • What do the November 2026 AI answer results show about National Guardian Life mentions, recommendations and placement?

National Guardian Life ranks 8 by recommendation count. Mentions: 18 (7.66%); recommendations: 12 (5.11%). Top 3 placements: 4 (1.70%); first-place placements: 0 (0.00%).

Wording about the company: 12 positive, 6 neutral, 0 negative mentions. The proportion of mentions that also included a recommendation was 66.7%. The net framing score was 0.67 (defined in Methodology).

6 mentions had no recorded recommendation. Of recorded recommendations, 33.33% were Top 3 and 0.00% were first-place.

From October 2026 to November 2026, National Guardian Life's mention count fell from 30 to 18, while its recommendation count fell from 30 to 12. The recommendation rate was 11.54% and 5.11% respectively.

National Guardian Life has a smaller footprint, and its recommendation-to-mention ratio is lower than most peers, with a notable share of neutral wording. Top-three placements are few and first place is absent. Both mentions and recommendations fell from the prior month; the smaller answer base is a separate fact and does not explain that decline.

Genworth

Questions This Section Answers

  • What do the November 2026 AI answer results show about Genworth mentions, recommendations and placement?

Genworth ranks 9 by recommendation count. Mentions: 11 (4.68%); recommendations: 4 (1.70%). Top 3 placements: 0 (0.00%); first-place placements: 0 (0.00%).

Wording about the company: 4 positive, 6 neutral, 1 negative mentions. The proportion of mentions that also included a recommendation was 36.4%. The net framing score was 0.27 (defined in Methodology).

7 mentions had no recorded recommendation. Of recorded recommendations, 0.00% were Top 3 and 0.00% were first-place.

From October 2026 to November 2026, Genworth's mention count rose from 6 to 11, while its recommendation count rose from 2 to 4. The recommendation rate was 0.77% and 1.70% respectively.

Genworth appears in few answers and is endorsed in only a minority of those appearances, with neutral wording outnumbering positive and one negative mention recorded. No top-three or first-place placements appear. Both mentions and recommendations rose slightly from the prior month, but the counts are small enough that the ratio should be read cautiously.

Knights of Columbus

Questions This Section Answers

  • What do the November 2026 AI answer results show about Knights of Columbus mentions, recommendations and placement?

Knights of Columbus ranks 10 by recommendation count. Mentions: 1 (0.43%); recommendations: 0 (0.00%). Top 3 placements: 0 (0.00%); first-place placements: 0 (0.00%).

1 mentions had no recorded recommendation. Of recorded recommendations, Unavailable were Top 3 and Unavailable were first-place.

From October 2026 to November 2026, Knights of Columbus' mention count stayed at 1, while its recommendation count stayed at 0. The recommendation rate was 0.00% and 0.00% respectively.

Knights of Columbus records a single mention, framed neutrally, with no recommendation and therefore no placements. The zero counts are measured results rather than missing observations; only ratios requiring a nonzero base are undefined. This profile describes one appearance in the selected answers and supports no broader inference.

Only months with verified results are shown. Monthly sample coverage is listed in Methodology.

Each month is a separate sample of AI answers, and the number of included answers differs across months. Counts and rates therefore move for reasons the table cannot identify, and a changed rank does not establish stability, improvement, or loss of standing. The snapshots show recorded results month by month, not a continuous series.

What the November Rankings Show

These measures answer different questions. Reach asks whether a company appeared; recommendation asks whether the answer affirmatively endorsed it; placement asks where it fell within an endorsement; framing asks how it was described. A company can be widely mentioned yet seldom endorsed, or endorsed often yet rarely placed first. None of these readings establishes why an answer included a company, nor do they measure customers, sales, or product quality.

Methodology

Questions This Section Answers

  • How were the November 2026 Long-Term Care Insurance AI visibility measures counted, and what do they not establish?

This benchmark has partial coverage: 3 of 3 planned question categories contributed usable observations; 0 contributed none. This describes question-category coverage, not demonstrated platform outages. Findings apply to the available answer set.

The month in the title is the reporting period. The exact dates when the original AI answers were collected have not been verified.

Each company is counted at most once in each answer that meets the benchmark rules. A recommendation requires both an affirmative endorsement and positive wording. Mention, recommendation, Top 3 and first-place rates use the full benchmark answer total. Top 3 and first-place results are included within recommendations, not added to them.

The recommendation-to-mention ratio divides recommendations by mentions. It describes AI answer wording, not customer conversion. Net framing is positive minus negative mentions, divided by all mentions, on a scale from -1 to +1. It does not measure customer satisfaction or company quality.

Market: US; 10 tracked companies. AI services represented: ChatGPT, Copilot, Gemini, Google AI Mode, Google AI Overviews, Perplexity.

The source does not provide a reader-facing definition of “reserved”; that screening label is reported without inferring a reason for exclusion. Only the included benchmark answers form the rate denominator.

One answer can mention or recommend several companies. Adding company totals would therefore count some answers more than once. The results describe the AI answers reviewed, not independently verified facts about the companies or their products.

The answer samples differ between months. These comparisons show how the recorded results changed; they do not establish why they changed.

Results cover the answers that met the benchmark rules, not every answer captured, and the exact collection dates are unverified. Each company counts at most once per answer, while one answer may endorse several companies, so company totals overlap. Recommendation requires affirmative endorsement plus positive wording. Rates use the full included answer total, and placements sit inside recommendations. Net framing is a wording score, not satisfaction. Monthly samples differ, so comparisons show recorded change without explaining it.
