UnitedHealth Group recorded a negative but still inconclusive change in AI recommendation coverage in the initial LLM Authority Index public-company panel.
Across 366 matched prompt-platform cells, UnitedHealthcare-related recommendation coverage declined from 64.5% in July to 59.8% in September 2026, a change of -4.6 percentage points.
The decline was broad across several AI systems, but it did not satisfy the full V0 negative-candidate rules. Four of six platform families worsened, one improved, and one was unchanged:
- ChatGPT: -11.11 pp
- Gemini: -21.88 pp
- Google AI Mode: +4.04 pp
- Google AI Overviews: -4.05 pp
- Microsoft Copilot: 0.00 pp
- Perplexity: -17.65 pp
The exploratory 95% interval ranged from approximately -9.5 to +0.3 percentage points, crossing zero narrowly on the positive side. The aggregate point estimate also stopped just short of the current -5 percentage-point threshold used for the V0 Negative AI divergence candidate label.
UnitedHealth Group therefore receives a Medium AI-measurement confidence classification and remains Mixed / neutral under the current watch rules.
Simple presence stayed extremely high. UnitedHealthcare-related entities appeared in 98.6% of eligible matched responses in July and 96.4% in September, a decline of only 2.2 percentage points. Average recommendation rank was also almost unchanged, moving from approximately 1.98 to 2.05.
This creates a useful measurement pattern: UnitedHealthcare remained nearly ubiquitous in the matched response set, but it was recommended somewhat less often across several AI systems while its average position when recommended was broadly stable.
That does not establish weaker medical membership, lower Medicare enrollment, softer commercial enrollment, lower premium revenue, weaker Optum results, lower earnings, or a negative stock outcome.
The prospective question remains the one defined by the AI Commercial Momentum Hypothesis: do persistent changes in unbranded AI recommendation behavior contain incremental information about future commercial outcomes after accounting for information already available when the signal was measured?

How Much Did UnitedHealthcare Recommendation Coverage Decline?
UnitedHealthcare-related recommendation coverage declined 4.6 percentage points, from 64.5% to 59.8% across the matched July-to-September panel.
The point estimate is meaningfully negative, but two aspects of the evidence prevent a stronger V0 classification.
First, the current negative-candidate threshold requires an aggregate decline of at least 5 percentage points. UnitedHealth Group stopped just short at -4.6 points.
Second, the exploratory interval ranged from approximately -9.5 to +0.3 percentage points. Because the upper bound remained slightly above zero, the row is still formally inconclusive under the current exploratory framework.
This is exactly the kind of borderline case the prospective design is intended to preserve rather than round into a stronger story.
A later outcome should not be allowed to retroactively change the September classification.

Why Does UnitedHealth Group Remain Mixed / Neutral?
The aggregate decline is notable, and four of six platforms worsened, but the V0 rules are intentionally mechanical.
A company is not upgraded or downgraded to a directional candidate because the narrative feels persuasive. The same criteria apply across all companies.
For UnitedHealth Group:
- the point estimate is -4.6 pp, just short of the -5 pp negative-candidate threshold;
- the exploratory interval crosses zero at +0.3 pp on the upper bound;
- four platforms worsened, which is broad but not universal;
- one platform improved and one was unchanged;
- presence remained above 96% in September; and
- average recommendation rank was almost unchanged.
The result therefore remains Mixed / neutral even though its aggregate direction and platform breadth lean negative.
That discipline matters for later validation. If a borderline negative signal is later followed by weaker commercial results, the historical record should still show that it was borderline and inconclusive at the time. If later results are strong, the record should not be rewritten to minimize the initial decline.

What Does Nearly Universal Presence Tell Us?
UnitedHealthcare-related presence was extremely high in both periods:
- July: 98.6%
- September: 96.4%
That means the tracked UnitedHealthcare brands appeared in almost every eligible matched response.
But recommendation coverage was materially lower:
- July: 64.5%
- September: 59.8%
This distinction is important.
Presence answers a basic visibility question: Was UnitedHealthcare mentioned or represented in the response?
Recommendation coverage answers a stronger commercial question: Was UnitedHealthcare actually recommended in the eligible response?
A brand can be almost universally present while still being recommended in a smaller share of responses. The initial UnitedHealth Group result shows exactly that pattern.

Platform Differences Were Material
UnitedHealth Group's aggregate decline did not occur uniformly across all six AI platform families.

The largest negative movements came from Gemini and Perplexity. ChatGPT also declined by more than 11 points.
Google AI Mode moved in the opposite direction, increasing 4.04 points, while Microsoft Copilot was unchanged.
That cross-platform disagreement matters because an aggregate number can conceal different recommendation ecosystems underneath it.
The cross-platform recommendation portability framework therefore treats breadth as a separate signal-quality dimension rather than assuming that every AI system should move together.
For UnitedHealth Group, the current interpretation is straightforward: the decline was broad but not portable across all platforms.
The Average Recommendation Rank Was Essentially Stable
Average recommendation rank changed only slightly, from approximately 1.98 to 2.05.
Because a lower numeric rank is better, that movement is a small deterioration. But it is far smaller than the change seen in recommendation frequency.
That means the initial UnitedHealthcare signal is primarily a coverage story, not a rank story.
When UnitedHealthcare was recommended, it remained positioned near the top of the recommendation set on average. The change came from being recommended in a smaller share of eligible cells, not from falling dramatically lower in the responses where it still appeared.
This distinction is commercially relevant because recommendation frequency and recommendation position represent different forms of competitive visibility.
They should not be collapsed into one opaque score before longitudinal testing shows which measurement, if any, is more informative about later outcomes.
The UnitedHealthcare Brand Mapping Is Not the Same as UnitedHealth Group Enterprise Exposure
This limitation is central to Article 26.
The V0 public-parent mapping includes:
- UnitedHealthcare
- UnitedHealthcare (Golden Rule)
- UnitedHealthcare Vision
The tracked entities are therefore consumer-facing UnitedHealthcare brands, not every part of UnitedHealth Group.
UnitedHealth Group itself describes the company as operating through two distinct and complementary businesses: UnitedHealthcare and Optum. UnitedHealthcare provides health benefits, while Optum includes care delivery, pharmacy, technology, and related health-services businesses.
Official company context:
The current AI signal should therefore be interpreted as UnitedHealthcare consumer-brand recommendation exposure rolled to the UnitedHealth Group public parent.
It should not be interpreted as a direct AI measurement of Optum Health, Optum Rx, Optum Insight, or every revenue stream inside UnitedHealth Group.
That distinction will be especially important when the signal is compared with later financial outcomes.
A consumer plan-selection signal may have a plausible relationship to UnitedHealthcare enrollment or member acquisition. It may have little direct relationship to a particular Optum service line.

Why the Parent-Exposure Problem Matters for Future Validation
A public-company investor signal becomes less useful if the measured consumer exposure is only loosely connected to the economics of the listed parent.
UnitedHealth Group is a strong test of this problem because the company is large, diversified, and operates major businesses outside the consumer insurance decision journey captured by the current prompt universe.
Future validation should therefore test multiple levels of outcome alignment.
More directly aligned outcomes
Potential UnitedHealthcare-aligned outcomes include:
- total medical membership;
- commercial membership;
- Medicare Advantage enrollment;
- individual and family plan enrollment;
- Medicaid membership where relevant to the measured prompts;
- branded search demand for UnitedHealthcare;
- plan-shopping traffic;
- quote, enrollment, or plan-comparison activity where available.
Less directly aligned parent outcomes
Parent-level outcomes such as total UnitedHealth Group revenue, consolidated earnings per share, or total stock returns are economically farther from the measured consumer signal because they include Optum and other business drivers.
Those outcomes can still be tested, but they should not be treated as if the AI measurement has a one-to-one relationship with the entire enterprise.
The parent-exposure problem is one reason the validation protocol calls for business-line alignment and economic exposure weighting rather than naive parent-company attribution.
What Was Already Known During the Signal Window?
Time ordering matters.
UnitedHealth Group reported second-quarter 2026 results on July 16, 2026. Those results, along with the company's updated full-year outlook, were public during the July-to-September AI observation window.

That information belongs in the historical baseline information set for any later predictive model. It must not be counted as a future outcome supposedly anticipated by the AI signal.
By contrast, UnitedHealth Group has announced that it will release third-quarter 2026 financial results on October 13, 2026, after the September signal freeze.

That creates a clean prospective checkpoint for the dated September observation.
The existence of that future checkpoint does not mean the AI signal predicts the Q3 result. It simply means the historical ordering can be preserved without hindsight leakage.

How UnitedHealth Group Compares With Other Healthcare Signals
Within the initial healthcare slice:
- Molina Healthcare increased 3.7 pp and remained Mixed / neutral.
- Cigna was exactly flat at 0.0 pp.
- UnitedHealth Group / UnitedHealthcare declined 4.6 pp and remained Mixed / neutral.
- CVS Health / CVS Pharmacy declined 9.4 pp and met the V0 negative-candidate rules.
- Labcorp / Labcorp OnDemand declined 10.0 pp but remained Exploratory because of the smaller five-platform panel.
UnitedHealth Group therefore sits between the stable/mixed names and the larger negative healthcare movements.
That relative placement is descriptive only. It is not a ranking of company quality, financial strength, or investment attractiveness.
How UnitedHealth Group Compares With the Insurance Slice
UnitedHealth Group also appears in the broader insurance and health-plan sector analysis.
The insurance slice contains both positive and negative observations. MetLife was the only positive V0 candidate. Principal and Molina were positive but mixed. Cigna was flat. Several insurers declined but remained mixed because their intervals or platform breadth did not satisfy the negative-candidate criteria.
UnitedHealth Group's -4.6 pp result belongs in that middle group: negative in point estimate, broad across several platforms, but not directional under the current rules.
This makes UnitedHealth Group especially useful for later validation. If only the most extreme positive and negative observations are ever evaluated, the research would miss whether intermediate signals contain useful information.
What This Does Not Mean
The initial UnitedHealth Group signal does not establish that:
- UnitedHealthcare membership will decline;
- Medicare Advantage enrollment will weaken;
- Medicaid membership will fall;
- commercial enrollment will slow;
- premium revenue will decline;
- medical-cost trends will worsen;
- Optum will underperform;
- UnitedHealth Group earnings will decline;
- analysts will reduce estimates;
- the stock will underperform; or
- the company is overvalued or unattractive as an investment.
The V0 result says something much narrower:
Across the matched July-to-September AI panel, UnitedHealthcare-related recommendation coverage declined 4.6 percentage points, with four of six platform families worsening, while presence remained extremely high and average recommendation rank remained broadly stable.
That is the observation being frozen for prospective testing.

What We Will Test Next
The strongest validation design will test whether the frozen AI signal adds information beyond traditional variables that were already observable on September 30, 2026.
Near-term commercial and demand measures
Potential tests include:
- branded search for UnitedHealthcare;
- plan-shopping traffic;
- UnitedHealthcare site engagement;
- Medicare plan-shopping activity;
- commercial and individual plan demand indicators;
- enrollment where timing and disclosure permit;
- consumer consideration proxies.
Financial and operating measures
Potential company outcomes include:
- UnitedHealthcare membership changes;
- premium revenue growth;
- segment revenue;
- operating margin;
- medical-care ratio changes;
- analyst revenue and EPS estimate revisions;

Parent-level market outcomes
Only after commercial relationships are tested should later stages evaluate:
- sector-relative stock returns;
- factor-adjusted returns;
- whether the AI signal adds information beyond prior financial performance, valuation, market momentum, earnings expectations, and known regulatory or company events.

Methodology Notes for the UnitedHealth Group Row
The V0 result is based on 366 matched prompt-platform cells grouped into 255 normalized prompt clusters across six platform families.
The aggregate recommendation-coverage change is computed over the matched panel so that July and September are compared on the same eligible prompt-platform population.
The exploratory interval is constructed from prompt-level mean changes and should not be interpreted as a formal causal confidence interval.
The V0 classification is Medium because the row has more than 100 matched cells, all six platforms, and cleaning sensitivities within the current Medium-confidence treatment, but the interval crosses zero and the result does not satisfy all High-confidence directional requirements.
Sensitivity results were:
- no-dedupe sensitivity difference: +1.29 pp;
- capture-average sensitivity difference: +0.69 pp.
Those changes do not reverse the overall interpretation, but they are large enough to keep cleaning sensitivity visible rather than presenting the point estimate as exact.

Limitations
Several limitations are especially important for UnitedHealth Group.
Brand-to-parent mapping
The tracked entities are UnitedHealthcare consumer brands, not the whole UnitedHealth Group enterprise. Optum is not directly represented by this signal.
Prompt-population limitation
The result depends on the commercial questions included in the source datasets. It should not be generalized to every healthcare or insurance decision.
Short observation window
July through September is an initial longitudinal window, not enough history to establish persistence or predictiveness.
Platform dependence
Gemini, Perplexity, ChatGPT, Google AI Overviews, Google AI Mode, and Copilot moved differently. The aggregate result should not be treated as universal across AI systems.
Outcome-validation limitation
No claim has yet been established that AI recommendation momentum predicts membership, revenue, earnings, analyst revisions, or stock returns.
Confounding
Health-plan recommendations may be influenced by geography, plan availability, network access, premium pricing, Medicare star ratings, employer benefits, regulatory changes, consumer reputation, product design, and other factors that may also affect commercial results.
These factors must be controlled or explicitly modeled before the signal can be interpreted as incremental alternative data.
Why This Observation Matters Even If It Later Fails
The research program is intended to create a dated record before outcomes are known.
UnitedHealth Group is valuable because it is not an extreme V0 case. The signal is negative but not quite directional under the current rules. Presence remains extremely high. Average rank is stable. Four platforms decline, but not all six.
If future commercial data show no relationship to this signal, that is informative.
If the signal repeatedly weakens while UnitedHealthcare commercial indicators remain strong, the AI recommendation measure may be commercially irrelevant for this company or prompt population.
If similar borderline declines across many companies later precede weaker consumer demand, the threshold itself may need re-evaluation.
The correct research approach is to preserve the observation now and let later evidence determine whether it has value.
